1. Duly accomplished and Notarized Unified Application Form and Ancillary Permits ( Civil/Structural, Electrical, Plumbing ,Architectural Permits, etc.) – 4 original copies, back to back
2. Certified true copy of OCT/TCT by the Registry of Deeds – Land Registration Authority (LRA) – 1 original, 3 photocopies
In the absence of any existing CTC/OCT in the name of applicant; a certification from the CENRO-DENR
A Certificate from City Housing (if City Housing Project)
3.Building Plans & Design, Bill of Materials & Estimates, Project Specifications (Sign and sealed by Designed professionals and signed by the owner/applicant) – 4 sets

Sanitary/Plumbing Documents
Electrical Documents
If applicable:

Mechanical Documents
Line and Grade (Geodetic Documents)
Fire Protection Plan
4.DOLE’s Construction and Safety and Health Program (CSHP) – 1 original , 3 photocopies
5.Photocopies of Valid Licenses (PRC ID) & Current PTR of all involved professionals with dry seal and specimen signature – 4 photocopies
6. For application filed by authorized representative : A sworn Special Power of Attorney for the representative to file/ follow-up/sign application and to claim decision on the application with valid ID – 1 original, 3 photocopies
7. Expanding Envelope (2 pieces)
Folder (1 piece/long)
8. The following requirements are to be submitted
if necessary:
Barangay Clearance
Height Clearance Permit from CAAP
(if the Structure/Building is within the flight path)
Road-Right-of-Way Clearance from DPWH
(if Structure/Building fronting to the National Road/Highway)
